PDS Preferred – New Vendors

Welcome and congratulations for being approved to become a PDS Preferred Vendor.  Please review the information and steps below for submitting your registration.

 

Must have all of the following to become a PDS Vendor:

  • Vendor Information Request Form (form follows)
  • Federal Tax Form W-9 (form & instructions follow)
  • Proof of Insurance – General Liability Certificate
  • Proof of Insurance – Worker’s Compensation Certificate
  • Current License with the Arizona Registrar of Contractors

 

INSURANCE INFORMATION:

Vendors must annually provide evidence of General Liability and Workers Compensation Insurance to the Association.

  1. The General Liability Insurance limit must be at least $1,000,000 for a single occurrence and name the Association as Additional Insured.
  2. When providing a Certificate of Liability Insurance as evidence, the Description of Operations Section must include a statement that; “(Legal Name of Association) is additional insured on the General Liability policy,” and the Certificate Holder or Additional Insured Section show:

    (Legal Name of Association)

  3. Evidence of Workers Compensation Insurance may be provided on a Certificate of Insurance or as copies of the current Declaration Pages.

Also, if you’re working at the HOA(s) on a regular basis please remember to keep your address, contact information, proof of insurance and licenses up to date with us at all time.
